PARTICIPANT WAIVER, ASSUMPTION OF RISK & RELEASE OF LIABILITY

Acknowledgment and Assumption of Risk

I understand that participation in the Party For Change Field Day is voluntary and may include physical and recreational activities such as relay races, tug-of-war, sack races, three-legged races, running, jumping, and other group games.

I understand that participation in these activities involves inherent risks, including but not limited to falls, collisions, slips, trips, muscle strains, sprains, broken bones, cuts, bruises, heat-related illness, adverse weather conditions, contact with other participants or equipment, and other injuries that may occur during physical activity.

I voluntarily choose to participate in the event with knowledge of these risks and accept responsibility for the risks inherent in my participation.

Participant Responsibility

I represent that I am physically able to participate in the activities I choose to undertake. I understand that I am responsible for determining whether an activity is appropriate for me and may decline or stop participating in any activity at any time.

I agree to follow reasonable event rules, safety instructions, and directions provided by Party For Change, event staff, volunteers, venue or park personnel.

I will not participate in an activity if I believe I cannot do so safely.

Release and Waiver

To the fullest extent permitted by applicable law, I release and hold harmless Party For Change LLC and its owners, officers, employees, contractors, volunteers, agents, event partners and sponsors from claims arising from the ordinary and inherent risks associated with my voluntary participation in the event.

I understand that this release does not waive any rights or claims that cannot legally be waived under applicable law.

Medical Treatment

If I become injured or require medical attention during the event and I am unable to provide instructions, I authorize event personnel to contact emergency medical services on my behalf.

I understand that Party For Change does not provide medical insurance for participants and that I am responsible for medical expenses I incur in connection with my participation, except where otherwise required by law.

Personal Property

I understand that I am responsible for my own belongings during the event. To the fullest extent permitted by law, Party For Change is not responsible for lost, stolen, misplaced, or damaged personal property.

Photo and Video Release

I understand that photographs and video may be taken during the event.

• YES, I give Party For Change permission to photograph and/or record me during the event and to use those images or recordings for Party For Change's website, social media, marketing, fundraising, press, and promotional materials without compensation.

• NO, I do not give permission for Party For Change to intentionally use identifiable photographs or recordings of me for promotional purposes.

I understand that Party For Change will make reasonable efforts to honor my selection but cannot control incidental appearance in wide-angle or crowd photography taken in a public setting or photographs taken independently by other attendees.

Event Conditions

I understand that outdoor events may be affected by weather, field conditions, public park activity, or circumstances outside Party For Change's control. I agree to comply with applicable park and venue rules.

Acknowledgment

By purchasing a ticket, I am accepting this agreement, and I acknowledge that:

• I have read and understand this Participant Waiver, Assumption of Risk & Release of Liability.

• I understand that participation in the Field Day is voluntary.

• I understand the nature and inherent risks of the activities involved.

• I voluntarily assume those inherent risks.

• I agree to the terms above to the fullest extent permitted by applicable law.

• I am at least 18 years old.
